Registration Forms are available in hard copy format through the church office (989-631-2031) or the Summer Camp display in the church foyer.

You may also download the Registration Form in PDF format which is viewed through Adobe Acrobat Reader. If you don't have the Adobe Acrobat reader you can download it from Adobe.com.
